Classroom Discipline & Behavior Management
Sample Behavior Contract (Word)
Depending on your students, you may want to implement a behavior contract to help keep your students accountable for the classroom and school rules. A behavior contract is a simple document with a list of rules and consequences. The consequences need not only be negative but also should include rewards for positive behavior. The advantage to a behavior contract is that when a student breaks a rule he cannot use the excuse that he does not know what the rule is. You would simply pull out your file full of signed behavior contracts and say that you have their signature acknowledging the rules.
Sample Class Rules (Word)
Of course most teachers come up with their own specific rules depending on the school’s rules and nature of their students, here are some sample rules that I use.
Family & Consumer Science Passes (Word)
I don’t know about you but it seems like students use every excuse possible to leave class and walk around the building. In attempt to eliminate that issue and keep students in class I started issuing a set number of passes per semester. I give each student six passes to leave the classroom. They may be used for getting a drink using the restroom. They are allowed to leave my classroom six times per semester as long as they use one of their passes. If a student does not have his passes with him that day he can choose to either remain in class or borrow a friend’s pass. To encourage students to stay in class I offer them one extra credit point for each pass they have left at the end of the course.

Test Review Strategies
Bingo (PDF)
Playing BINGO is a fun way to review for an exam that possibly has many terms or facts to remember. One way to do it is to have students write the terms on the blank bingo card and you read off the definitions. The student then has to match the definition to the correct term on his/her bingo card. To have this work correctly you must have more terms with definitions then squares on the bingo card.

Extra Credit
Culinary Extra Credit Form (Word)
This is a form for students to fill out for any recipe that they make at home. Of course I have to specify that it has to be a recipe with at least five or more steps and cannot be from a box or mix. I was hesitant to ask for a picture at first because I didn’t want to leave anyone out that didn’t have a camera but I have found that almost all cell phones have cameras on them these days and that most students are able to easily comply.

I love all the great ideas! Thanks
I agree eemiller2.
You can also us e a receipt book for behavior. If a student has bad behavior write them a "ticket". You have a hard copy in our book & you give the students a copy. You sign the book as well as the student. This way… when you call home to "mom"… you tell Mom that Lil Judy had been told her behavior was not accepable in the past… See they even signed the book & got a copy of the ticket.
Does anyone know where I can get a pre-made list of warm ups/bell ringers/bell work for middle school FAC? I have only been able to find one book, but it didn't work with what we were doing.
